DOCUMENTARY REQUIREMENTS TO QUICKLY PROCESS CLAIMS
For us to quickly process your claims request, the following documents are needed:
A. VEHICLE INSURANCE CLAIMS
OWN DAMAGE
- Police / Incident Report
- Notarized affidavit of the Insured’s driver
- Clear copy of OR/CR of the insured vehicle
- Driver’s license with OR
- Repair estimate preferably from accredited shop of insurance provider
- Photograph of damage portion showing the plate number of the insured unit
IF THIRD PARTY DAMAGE
Other requirements apart from the above given list.
- Clear copy of OR/CR of both the insured and third party vehicles
- Driver’s license with OR of both the insured and third party drivers
- Certificate of No Claim of the third party vehicle
- Photograph of damage portion showing the plate number of both the insured and third party vehicles
B. ENGINEERING INSURANCE CLAIMS
- Contractors All Risk
- Duly accomplished Sworn Statement of Claim
- Duly accomplished Non-Waiver Agreement
- Police / Incident Report
- Technical Report/Assessment as tthe cause of loss
- Copy of Contract Agreement with Bid Estimates
- Copy of Notice of Award
- Copy of Notice of Commence
- Copies of Business Registration, permit, etc.
- QUICK CLAIMS
- Survey of damages based on standard engineering practices (Plans, survey data, PERT
- CPM, etc.)
- Projected Cash Flow of the project
- Detailed computation of loss and damages in order tbring back the damaged portions to
- its original condition before the loss
- Certified copies of recent and previous monthly accomplishment/progress report including its
- back up computation
- Certified copies of recent and previous billing reports
- Actual cost of repair/replacement of the affected properties such as invoices, receipts, etc.
- Clear and properly annotated panoramic pictures immediately taken after the loss
- Damaged Electronic Equipment

C. PROPERTY INSURANCE CLAIMS
- General Requirements
- Police / Incident Report
- Photographs depicting the damages
- Building Claim
- A Civil Engineer’s or Architect detailed estimate for the restoration and/or repair of the damaged building
- Certified copy of building permit
- Copy of the building plan and/or sketch complete with dimensions/specifications
- Certified copy of declaration of real property (Lease contract or agreement if the insured is not lot owner)
- Machinery/Equipment/Furnitures/Fixtures/Fittings Claim
- A complete description/specification of the affected and unaffected items as of the date of the loss, date acquired, year, model
- Actual cost of repair/replacement of the affected properties such as invoices, receipts, etc
- Stocks Claim (Goods & Merchandise)
- Books of accounts, Financial Statement, Purchases and Sales Invoices and all otherbusinesses records
- Inventory of affected and unaffected stocks as of date of loss indicated the quantity and cost
- Certified copies of Income Tax Returns of the Insured for the last two fiscal years
- Latest inventory of merchandise filed with the Bureau of Internal Revenue or any government entity prior to the loss
D. MARINE INSURANCE CLAIMS
- CARGO
- Duly receipted formal claim against Vessel, Arrastre, Broker and Forwarder
- Original Marine Cargo Policy/Risk Note
- Original Bill of Lading/Master Air Waybill
- Original Packing List
- Original Commercial Invoice
- Original copy of delivery receipts
- Photographs taken at the time of the loss
- Other documents which may be required to quickly evaluate your claim
- IN LAND / TRUCK RISK CLAIMS
- Police / Incident Report
- Invoice, Purchase Order, Valued Packing List or any documents showing the actual value ofthe cargo
